New Patient Registration

The Practice welcomes new patients

registrationWhen you register with us you will need to complete a registration form which is available at reception or you can download here. This form is for patients who are permanently living in the area. We also require new patients to complete the New Patient Questionnaire which is also available at reception or you can download here. Please complete these forms and post them to the practice or hand in at reception.

On the online medical questionnaire we will request details of your past medical problems so as to provide you with appropriate medical care while we wait for your records to reach us. Your previous GP records can take anything up to three months to arrive at the practice. Even in these days of coded computerised medical records, there is as yet no way of transferring the electronic record from practice to practice.

Temporary residents (those on holiday or here on business temporarily) would need to register as a "temporary resident" using a different form available at reception or by downloading it here.

Register Before You Need Us!

Many new patients only register with us when they have a medical problem. We strongly recommend that you register when you move into the Practice area.

New Patients

At the time of registration, patients over 12 years and 0-12 year olds with chronic illnesses are encouraged to make an appointment for a 'new patient check' with the practice nurse before needing to consult a doctor. This enables much of the information regarding past medical problems to be recorded and acted upon as necessary. At that appointment the nurse will be able to review the new patient questionnaire completed at the time of registration. If you are attending for a new patient check with the practice nurse, you may find it more convenient to bring a fresh urine sample with you rather than produce one at the time. Our Receptionists can provide you with a sample pot and can answer any further queries you may have.
